Katherine Alconcel, DO
Medical Director

Dr. Alconcel has been with Vituity since 2016 and serves as a Medical Director for Jovive Health in Henderson, NV.

Marcella Jaeger, MD

Dr. Jaeger has been treating patients in the urgent care setting for 22 years.
Prior to urgent care, she served as a medical doctor in the U.S. Navy.

Sadeeq Sadiq, MD

Don Rose

Dr. Rose has extensive knowledge and work experience in emergency medicine, sports medicine as well as acute care medicine. He has taken care of patients for over 30 years in emergency and urgent care medicine.

Dany Boulos, MD
Medical Director
He attended medical school at Al Baath University in Syria, completed a Family Medicine residency program, and became Board-Certified in Family Medicine in 2012.
Dr. Boulos practiced as a Family Medicine Doctor in Terre Haute, IN from 2011-2015. He then spent two years working in Urgent Care at Temple University in Philadelphia. He joined Vituity in 2017, and continues to enjoy working as an Urgent Care Doctor.
When treating patients, Dr. Boulos discusses decisions with patients and applies the most updated medical information in order to meet the patients’ goals.
In his free time, he loves to spend time with his wife and two daughters, plays chess and watches soccer.
Eric Mallett, MD
Dr. Eric Mallett is an Urgent Care and Family Medicine doctor at Jovive Health Urgent Care. He also practices medicine at the Advocate Sherman Outpatient Clinic in Algonquin, IL. He received the 2018 Service Award for highest Press Ganey patient satisfaction scores at the Advocate-Aurora Sherman Hospital, Annual Physicians Recognition Gala.
Dr. Mallett earned his medical degree at the Medical School, American University of the Caribbean School of Medicine and completed his residency in Family Medicine at Providence Hospital and Medical Center, GME.
He has practiced Urgent Care, Workman’s Health, and Occupational Health for more than 10 years.
In the Urgent Care setting, Dr. Mallett has the privilege to care for patients with a wide range of needs. His training in Family Medicine uniquely positions him to address the care of patients of all ages and acuity in the outpatient setting.
When not at work, Dr. Mallett enjoys spending time with his family, attending sporting events, and eating at great restaurants. He is also interested in health, fitness and exercise.
Dany Mamou, MD
Dr. Dany Mamou graduated from The University of Aleppo, Faculty of Medicine in Syria with his MD. He then completed his Family Medicine Residency training at the University of Oklahoma in Tulsa, and has been working as a Family Doctor and Urgent Care Doctor ever since.
When practicing medicine, Dr. Mamou’s goal is to provide the best possible care to each patient, no matter how simple or complicated their concern. He works to stay up-to-date on medical advances and reviews literature daily so he can continue to provide excellent medical consults. He takes a collaborative approach to care, and provides options for treatment so that the patient can make the best choice for their unique lifestyle and condition.
Dr. Mamou is a family guy, and loves to spend any free time with his loved ones.
Kevin Wakeman, MD
Dr. Kevin Wakeman has worked full-time as an Urgent Care physician since 2001. He earned his MD at the University of Illinois, College of Medicine right here in Chicago.
In his practice, Dr. Wakeman believes in treating all patients as if they were family.
In his free time, Dr. Kevin enjoys the outdoors.
Emily Ellis, PA-C
Emily Ellis is a caring and compassion Physician Assistant. She enjoys the acute care aspect of medicine and being able to help people in times of need. Emily earned her Master of Science in Physician Assistant (PA) Studies at Indiana State University. She worked in hospital medicine for 3 years and transitioned to emergency medicine for the past 3.5 years. In her patient care, she values listening to the patient and educating them on medical decisions.
When not at work, Emily enjoys working out, and you might find her running, biking, or weight lifting. She likes to spend time with friends, especially outside on patios. She and her husband have 2 big french mastiffs.
Amy Forsberg, PA-C
Amy Forsberg is a thorough, caring, and experienced Physician Assistant. She has worked in both Urgent Care and Emergency Room settings throughout the Chicagoland area for the last 7 years. She enjoys the fast pace of emergent medical care, and is proud to provide comfort and care to patients in their time of need. Amy is honored to work in the medical field and help patients in these uncertain times.
Amy first trained as an EMT after her undergraduate studies. She went on to PA school in Pittsburgh and completed her Emergency Medicine Rotation in DC. Since graduating, she has been back in Chicago working in the Emergency Room and Urgent Care setting.
Amy works to treat patients in a manner she would like her friends and family to be treated. She makes time to sit down and truly listen to concerns.
In her free time, she enjoys exercising, being outside, and spending time with family and friends while trying new things in Chicago. Amy was a Division I College swimmer.
Lindsey Soderlund, PA-C

Joshua Zalot, PA-C
Joshua was born and raised in the Philadelphia area. He completed a Bachelor’s degree in Kinesiology at Temple University and worked as a Patient Transporter/ED Tech before pursuing a Master’s degree at DeSales University. With his PA Degree in hand, he moved to Gary, IN to work in Emergency Medicine at a busy community hospital.
When Joshua is not working, you can find him outside in the sun, going for a run on Lakeshore Drive, hanging with friends, or finding the new patio hot spot in Chicago for a nice craft beer. He also plays soccer in a recreational league for fun.
Crystal Medina, NP
Crystal loves the surprise element of emergency medicine and the unique challenges that it presents on a daily basis. In her practice, she works to educate patients in the management and prevention of conditions, and to bridge the gap between patients and other departments.
Crystal is happily married, and she and her husband Scott have the best dog- Bailey! She enjoys running, music and staying active during her time off.
